Je n'ai pas dormi ou mangé OR Je n'ai ni dormi ni mangé?

Should this be "Je n'ai ni dormi ni mangé"?

In other words, does 'ou' always change to a ne-ni-ni formation when used with negated clauses?

Can you say this, for example:
Elle a bu un caf
é, et/mais elle n'a pas travaillé ou fait du jogging.

It feels wrong to me, but I'd like to know if it's possible, and if not then what would be the correct form?

Yes you're right you are going to use "ni" instead of "ou" for a negative sentence. So it's not correct to say "elle n'a pas travaillé ou fait du shopping", you'll have to use "ni": "elle n'a ni travaillé, ni fait du shopping".
